The purpose of the tender is to develop a framework for service providers which will then be used to call off the delivery of a Complex Needs Pathway.

1 Specialist Nurse-led (LD or MH) Residential Placement with a Specialist MDT Team

For individuals stepping down from a secure hospital or stepping up from a community setting.
2 Residential Placement with Access to MDT

Short/medium term transitional placement that does not require a nurse led service.
3 Enhanced Supported Living with Access to MDT

24-hour, higher end supported living with access to a specialist MDT.
4 Enhanced Supported Living

24-hour core service core of cluster model.
