Description

The York Integrated Community Frailty Service provides proactive health management to support individuals in maintaining their independence; and provides integrated crisis care to frail and vulnerable individuals experiencing an acute health crisis in the community.

NHS Humber and North Yorkshire ICB is intending to award a contract an existing provider following direct award process C of The Health Care Services (Provider Selection Regime) Regulations 2023. Approximate lifetime value of the contract is £6,854,938 Contract term - 1 April 2025 - 31 March 2026 (the contract term may be extended by 2 x 6 month periods).
