Provision of Patient Discharge Transport services for the patients registered with GP Practices in Hambleton, Richmondshire and Whitby, which due to their acuity require specialised transport vehicles following discharge from the County Durham & Darlington NHS Foundation Trust (CDDFT) hospital (acute and community sites).
