Description
The Lewisham NHS@Home Service is a healthcare delivery model designed to provide comprehensive and coordinated care for patients outside of traditional hospital settings. It leverages technology, remote monitoring, and a multidisciplinary approach to manage the health and well-being of individuals, particularly those with chronic or complex medical conditions. The objective of NHS@Home is to reduce pressure on the acute healthcare system in Lewisham. This is achieved by 1) avoiding admissions and presentations to the emergency department, and 2) by facilitating supported discharges for patients already in the acute healthcare system. The target population are Lewisham residents, over the age of 18, who would otherwise require hospital level intervention. It is assumed that the majority of patients accessing the service will be within the elderly, frail population.

This is a Provider Selection Regime (PSR) confirmation of contract award notice; the original contract award notice was published on Find a Tender, notice identifier 2025/S 000-010623. This contract has been awarded under the Health Care Services (Provider Selection Regime) Regulations 2023. For the avoidance of doubt, the provisions of the Public Contracts Regulations 2015 do not apply to this award. This contract has now been formally awarded using direct award process C. The approximate lifetime value of the contract is £1,187,554 (for the term of one year). The contract term is for the period 1st April 2025 to 31st March 2026 (with break clauses at 6 and 9 months). The date of conclusion of the contract is 18th February 2025.
