The Dementia Support Service will include the following elements: • Support: o including assessments, goal setting, regular reviews, face to face support and crisis support. o Shorter term specialist support for people with memory loss, MCI or on the diagnosis pathway. This may be ongoing dependent on needs. o Support during transition to care home • Information and signposting • Education and awareness • Support and provision of Memory and Peer Support groups • Building networks and embedding into the system • Research
