Croydon Council and South West London ICB (Croydon) are working collaboratively to shape an exciting, new future opportunity to develop our three PFI Adult Care Homes for Older People in the borough with a range of needs, including Dementia, Learning Disabilities and Mental Health. There are currently 150 beds with a financial envelope of approximately £7M annually. Agenda: An opportunity to meet and network with a wide range key stakeholders involved in this change making initiative including directors of Adult Social Care, clinicians and other staff from community and care services and the commissioning project team; Current health and care landscape in Croydon - challenges and opportunities SWL frailty and Croydon's communities and neighbourhood models
