Description
The Supplier will be contracted to provide Social prescribing to children and young people aged 11-25 yrs to help improve health and wellbeing. The supplier will provide experienced link workers who will support each young person to create a personalised plan and set goals based on what is important to them. :EG To access support servicesTo access hobbies, activities and voluntary opportunitiesTo learn techniques to support their wellbeing whilst waiting for specialist supportThey offer 1-1 and group health and wellbeing sessions where in-person social prescribing assessments are offered. This contract will be for one year with the option of extending by a further year
