Description
The London Borough of Hammersmith & Fulham (the Council) is inviting suitably qualified and experienced providers to tender for the delivery of Public Health Nursing and Maternity in the Community Services. This procurement supports the delivery of the Healthy Child Programme (HCP) and aims to improve health outcomes for children, young people, and families across the borough. The services will be procured in three distinct lots: Lot 1: Health Visiting Service (0–5 years) Lot 2: School Health & Wellbeing Service (5–19 years, and up to 25 for young people with SEND) Lot 3: Maternity in the Community Service (0–2 years) Providers may bid for one, two, or all three lots. Each lot will be evaluated independently.
Lot Division
| 1 | Health Visiting for Children Aged 0–5 This lot covers the delivery of the Healthy Child Programme (HCP) for children aged 0–5, including the five mandated universal health and development reviews. The service will operate across four levels (community, universal, targeted, and specialist) ensuring proportionate support based on family needs. Health visitors will play a key role in identifying needs early and supporting families through the six early years high impact areas. |
| 2 | School Nursing for Children and Young People Aged 5–19 (up to 25 for SEND) This lot focuses on delivering school nursing services across local authority-funded schools and academies, aligned with national guidance. The service will provide support across four levels, with safeguarding embedded throughout. School nurses will work to improve outcomes in six high impact areas for school-aged children, including resilience, healthy lifestyles, and support for complex health needs. Delivery will also include the National Child Measurement Programme (NCMP), contributing to local and national public health intelligence. |
| 3 | Maternity in the Community Service This lot enhances the HCP through community outreach and supporting families from pre-birth to age two. The service will prioritise the six early years high impact areas, offering tailored support such as enhanced antenatal care, breastfeeding support, maternal mental health support, and health education. By integrating with existing services and co-locating within Family Hubs, the model aims to strengthen pathways and ensure equitable access to preventative and early help services. |
