Description
The Integrated Wellness Support Service (IWS) is a proposed model designed to improve health outcomes and reduce inequalities across Bolton by combining specialist stop smoking support and adult weight management into a single, holistic service. It will provide a single-entry point for residents seeking lifestyle and behavioural support, while also addressing wider determinants of health such as emotional wellbeing. The service will adopt a household approach, working in partnership with local neighbourhood models and community assets to deliver flexible, person-centred interventions. Delivery will include community-based and digital options, ensuring accessibility and alignment with Bolton's vision to improve health and wellbeing for all age groups.
Total Quantity or Scope
The service will deliver three levels of support: Universal Offer includes community-based health promotion, healthy eating guidance, physical activity initiatives, emotional wellbeing support, and signposting to local resources. It aims to build health literacy and encourage self-care strategies. Targeted Support provides structured one-to-one and group programmes for individuals needing more intensive help, including rehabilitation for long-term conditions and behavioural interventions linked to NHS Health Checks. Specialist Services focus on evidence-based interventions for weight management and smoking cessation. Weight management support will follow NICE guidance, offering structured behavioural programmes for adults with a BMI of 25-39.9 (or 23+ for ethnic minority groups). Stop smoking services will deliver behavioural support, pharmacotherapy, and harm reduction strategies, including NRT and vapes, in line with NICE guidance. Key deliverables include a high-profile universal programme, targeted interventions, a specialist weight management service, and a community stop smoking service with multiple referral routes. The provider must ensure accessibility, maintain strong partnerships with local health, care, and community networks, and operate flexibly to meet evolving needs. Performance will be monitored through KPIs such as quit rates, BMI reduction, and sustained behaviour change, with follow-up at 6 and 12 months. The service also requires robust safeguarding, data protection compliance, and trained multidisciplinary staff to deliver interventions safely and effectively.
