A Solution to support GP practices, PCNs and other users in delivering Health Checks within the community to assess Eligible Patients' CVD risk and support the operational processes and monitor the performance of the programme.

Renewal Options

The contract will Go-Live on 1st April 2027 and run until 31st March 2030 with the option to extend for up to a further 2 periods of 12 months (3+1+1), at the sole discretion of the Council.
