Description
The Stoke Association Life After Stroke (SA-LAS) Service offers key worker to provide ongoing, personalised interventions tailored to individual needs. These services include one-on-one sessions, group workshops, and vocational support to help survivors return to work and signposting. This service is an important service for survivors of stroke, and a service review suggests that the service does more than is currently reported including enabling people to adapt to modifications to enable them to return to work and reduce dependence on the NHS and adult social care services, through enabling access to support services to maintain independence.

The approximate lifetime value of the contract is £243,740 over two years on a one plus one year basis.
