Description

To provide a dedicated outpatient and domiciliary based management service to patients with complex postural and physical needs which do not fall within the scope of other NHS commissioned services

Total Quantity or Scope

The Chaseley Trust will offer an outpatient and Domiciliary Physiotherapy Service delivered and / or supervised by a Specialist Neurological Physiotherapist with expert knowledge and management skills of neurological conditions, complex needs and postural management. A patient will attend the Chaseley Trust for assessment via a referral from ESHT out-patient department or community-based rehabilitation team. There will be some patients who will require ongoing long-term maintenance due to their clinical presentation and diagnosis. These patients will be reviewed regularly (at least every 3 months) by a qualified physiotherapist who will thoroughly re assess the patient and their needs.
