Description

1.5 weekly GP sessions as face to face drop ins co located at The Friary Day Centre in West Bridgford. The cohort presents with complex SMD needs, including individuals excluded from Nottingham City areas due to probation restrictions or safety concerns. Holistic medical assessments are provided, addressing urgent presentations e.g. infections and co morbidities e.g. respiratory conditions, mental illness.

The ICB is intending to award a contract an existing provider following direct award process C The approximate lifetime value of the contract is £37,200 The dates between which the service will be provided are 1 April 2026 to 31 March 2028
