Description

The NHS Health Check programme (HC) is a 5-yearly contract with health care for adults aged 40-74 without a diagnosis of heart disease, kidney disease, stroke, or type 2 diabetes to manage Cardiovascular Disease Risk Factors (CVDRFs) for those conditions. A Health Check consists of an assessment of an individual's risks of developing a set of diseases in the future. This requires collecting a range of health information such as height, weight, smoking history, physical activity, blood pressure, and arranging for a cholesterol blood test to be performed. This is a statutory service legislated in 2012. In Stoke-on-Trent, the programme was suspended over the pandemic and continues to recover following pandemic disruption but is unlikely to achieve the national target of health check delivery to 75% of the eligible population. In Stoke-on-Trent, the programme is delivered exclusively by general practice as a universal offer. We are looking to expand the delivery of the programme and are therefore, seeking Providers who can deliver NHS Health Checks in non-healthcare settings, working with communities and using a targeted approach to reach areas with a low coverage, high health needs, or low uptake of offers. Providers will be expected to: • Host approximately 30 community events across the city per year. • Deliver 30-200 health checks at each event so that approximately 3,000 health checks will be delivered within this community outreach service per year. • Maintain their own record system and complete an agreed data template to be shared with the Locality Authority and the individuals registered general practice. As many of the benefits of Health Checks are realised through follow on services, Providers must demonstrate: • Strong knowledge and experience of referral pathways. • Relationships or the capability to develop relationships with communities to deliver health checks in settings that will reach target populations. • Flexibility in delivery approaches to encourage uptake of the health check, for example through utilising and continuously improving varied invitation methods. • Capacity to follow up with individuals requiring lifestyle interventions. Please note: The payment method will be via hybrid method consisting of a lump sum set up costs for each event/venue and payment by delivery
