Description
Cardiff and Vale University Health Board (CAVUHB) acts as the lead organisation for a long established Consortium comprising: Aneurin Bevan University Health Board (ABUHB) Cwm Taf Morgannwg University Health Board (CTMUHB) Hywel Dda University Health Board (HDUHB) Powys Teaching Health Board (PTHB) Swansea Bay University Health Board (SBUHB) The Consortium, formed in 2011, was established to secure a safe, specialist and sustainable model of planned, emergency and end of life palliative respite care for children with life shortening or life limiting conditions across Wales. This includes the provision of hospice stays for symptom management, crisis and emergency respite, end of life care, alongside community based and home based support to meet individual clinical needs. The service model ensures that children and their families receive coordinated, compassionate care throughout the child’s life and, where needed, into bereavement. Tŷ Hafan is the only provider in Wales able to deliver this integrated combination of specialist paediatric palliative care across hospice, home, hospital, and community settings. As a dedicated children’s hospice, Tŷ Hafan offers expert clinical support to children and young people with complex, rare or undiagnosed life shortening conditions, ensuring care that is fully personalised to their physical, emotional and social needs. For the reasons detailed above the intention is to award to Tŷ Hafan as the existing provider following Direct Award Process 1. The contract will be for a period of 3 years with the option to extend for a further 2 years in 12 months increments. It is estimated that the contract will be to the value of approximately 1.8millionGBP including the extension options.
