Description

Worcestershire County Council currently commission 12 extra care schemes across Worcestershire in partnership with the Housing Providers. In some schemes, the housing provider and the care provider are separate organisations, while in others, the housing provider also delivers the onsite care. An Extra Care Scheme is designed for individuals, typically aged 55 and over, and it combines independent living with access to 24/7 support and care services. It provides self-contained accommodation and offers additional services such as personal care and domestic assistance tailored to the individual’s needs. The goal is to maintain independence and quality of life by ensuring necessary support is readily available. The model integrates housing management, 24/7 support and care and is key for people to live the most independent and best life they can. There are two specific areas of support: 1. Onsite care for individuals - this is personal care which everyone who lives in the scheme may or may not need. This personal care tends to be the needs assessed by Adult Social Care. 2. Core support – this is for people living within the scheme which is a combination of housing, support and domiciliary care. This core support is available to everyone living within the extra care scheme. It is low level support to help people maintain their tenancy, to access opportunities within the scheme and in the wider local community such as leisure activities, social networking, or lifelong learning. The Council is seeking a supplier to provide the two specific areas of support above at one of its extra care schemes at Gilbert Court.

Award Criteria

Quality criterion 1 (Lot 0) Q1. Safe, Compliant & High-Quality Care Delivery Explain your integrated approach to delivering safe, compliant, and high-quality care in an Extra Care setting. Your response must include: • Compliance with required contractual and CQC standards • Safeguarding systems - risk identification, escalation, and review processes including lesson learned • Moving and handling people - training, competency assessment and supervision • Medication management – training, competency assessment and supervision • Management of any accidents, incidents, near misses, fire safety, and lessons learned • Please provide an example of an incident and how your organisation dealt with or would deal with this and any lesson learned Word limit: 1500 words
Quality criterion 2 (Lot 0) Q2. Person-Centred Care, Dignity, Outcomes & Tenant Experience Describe how you will ensure tenants consistently experience dignified, respectful, person-centred and strengths-based care. Your response must include: • How person-centred and strengths-based care is embedded in organisational culture, training, and daily practice • Supporting tenants with cognitive impairment, communication needs, sensory needs or fluctuating capacity • How outcomes are measured • How tenant experience is measured and improved (feedback, complaints, co-production) • A clear, plain English example which demonstrating the impact of person centre, strengths-based care Word limit: 1500 words

Responsive, Resilient & Safe Service Operations in Extra Care Explain how you will deliver a responsive and resilient service that always maintains safety and continuity. Your response must include: • Business continuity planning if there are any service disruptions • Managing planned and unplanned care and support with an extra care scheme • Risk identification, mitigation, and resilience assurance • Clear description of emergency response processes (e.g. alarm activation, including night-time response) • Evidence from a previous incident (or what you would do) if there was an incident and how this led (or would lead to) service improvements Word limit: 1000 words
Quality criterion 4 (Lot 0) Q4. Workforce Capability, Organisational Culture, Values & Partnership Working Describe how you will build and sustain a competent, confident workforce. Your response must include: • Recruitment, retention, and workforce planning strategy (including ensuring you have staff to meet current needs including changing or increasing needs) • Training, competency assurance and continuous development of staff • Culture and values of your organisation • Effective relationships and communication with the landlord, adult social care teams, health (including the mental health team) and other key stakeholders • Managing challenging needs and working collaboratively in the least restrictive way. This is to enable people to continue to live in their home in the extra care scheme for as long as possible (where safe to do so). Quality criterion 5 (Lot 0) Q5. Mobilisation, Service Delivery & TUPE Transition Explain how you will mobilise the service and work collaboratively with key partners to deliver an effective Extra Care service. Your response must include: • Mobilisation plan (including first 90 days, milestones, risks, dependencies) • Approach to TUPE transfer and staff engagement • Ensuring safe service commencement and continuity of care • Governance and communication with the landlord and adult social care and other key stakeholders • Managing shared responsibilities (e.g. building safety, accessibility) • Evidence from previous successful mobilisation of extra care or similar service Word limit: 1000 words

Lived Experience & Everyday Care (People with lived experience of extra care evaluation question) We want to understand what it will feel like to live in an extra care scheme with care provided by your organisation. Please describe, in clear and plain English language, how you will make sure tenants feel safe, respected, and well cared for every day. Your response must include:: • What happens when a tenant needs help (for example, if they use an alarm or call for support, including at night) • How staff will treat tenants with kindness, dignity, and respect • How you will get to know each person and support their individual needs and preferences • How you will listen to tenants, their families and unpaid carers and make improvements based on feedback • An example of what good care will look and feel like in day-to-day life Please avoid jargon or technical language. Quality criterion 7 (Lot 0) Q7. Record Keeping and Information Management Describe how you will ensure high-quality information management and use data to improve outcomes for tenants. Your response must include: • Systems and processes for accurate, timely, and secure record keeping • Data governance, quality assurance, and audit arrangements • How information will be shared with Worcestershire County Council, housing providers, and health partners • How data is used to support decision-making, risk management, and continuous improvement • How you collect feedback from tenants, people receiving care and staff and report on outcomes • Evidence where your approach has improved outcomes in similar services Word limit: 1000 words
Quality criterion 8 (Lot 0) Q8. Social Value Describe how you will deliver measurable social value through this contract.
