Description
The All Carers Service provides a citywide offer of information, advice, assessment and practical support for unpaid carers of all ages in Liverpool, including adult carers, young carers and parent carers. The service aims to identify carers early, assess needs, promote wellbeing, improve access to services and community support, and enable carers to sustain their caring role, including support with breaks, peer support, training and signposting, in line with statutory duties and local priorities.

| 1 | Digital Care Hub and Triage A citywide, single access point for carers of all ages through a digital platform and hub. Its core functions include: - Information, advice and guidance (IAG) for carers. - Initial triage and navigation to the right service, pathway or support - complementing and interfacing with existing mechanisms. - Supporting consistent data collection, performance reporting and outcomes tracking. - Improving the overall carer journey by reducing duplication and confusion |
| 2 | Adult Carers inclusive of Parent Carers Support for carers aged 18+ in a single, integrated adult carers offer, including parent carers. To provide: - Referral management to support the digital Triage system. - Statutory assessment and support planning function. - Provision of information & advice on carer rights . - Wellbeing activities. - Targeted outreach to diverse communities. - Appropriate co-location with other elements of the service to provide a seamless service offer and targeted outreach. |
| 3 | Young Carers, Young Adult Carers and Sibling Carers Support for carers aged up to 25. To provide: - Referral management to support the digital Triage system. - Statutory assessment and support planning function. - Provision of information & advice on carer rights . - Alignment with Children's Services and Education pathways to complement education, wellbeing & family life and support transition through life stages. - Age appropriate, tailored activities, and peer support opportunities. - Targeted outreach to diverse communities. - Appropriate co-location with other elements of the service to provide a seamless service offer and targeted outreach. |
| 4 | Carer Advocacy Independent advocacy for adult carers aged 18+, providing: - Referral management to support the digital Triage system. -Help to Carers through information & advice on their rights and to help them to understand their options. - To help carers to feel heard in assessments, reviews and decision‑making. - Help carers to navigate complex health, social care and wider systems. - Appropriate co-location with other elements of the service to provide a seamless service offer. |
| 5 | Carer Opportunties Flexible community-based opportunities for Carers. The contractor will: -Engage local businesses, community organisations and partners to source free or subsidised opportunities and activities for carers of all ages. -Articulate opportunities for business to deliver social value to the city and local communities. -Promote wellbeing, social connection and prevention. -Provide equitable access across all age groups. -Utilise and interface with technology/systems which deliver the digital platform. |
Renewal Options
Extension period of up to 12 months, subject to the provider satisfactory meeting contract performance measures within the contract.
