The Substance Use Outreach Nurse team works closely with Housing, Safeguarding, Primary Care, Pharmacies, and other colleagues, to take high-quality, outreach nursing care directly into local communities to support people experiencing homelessness and substance use.

The contract has been award following the Provider Selection Regime Option A. The contract value is £80,000 per year. Contract term is 31/03/2026 - 01/04/2027 with options to extend by 4 x 12 months

Legal Justification

The contract has been awarded via the Provider Selection Regime - Option A. The justification is because they are the only provider who can deliver this service from a technical/partnership perspective, as they have direct 'internal' links with each GP practice/Primary Care Network as well as access to patient healthcare records.
