Description
Hywel Dda University Health Board (HDUHB) have an opportunity to expand their provisions of Bereavement Services for the North and South Ceredigion Cluster Areas and are looking to commission a suitable provider to undertake this work for the County’s population. The contract will run for an initial two (2) year period, and have an option to extend for a further twelve (12) months.
Total Quantity or Scope
The successful provider will need to be able to communicate and liaise with our population through the medium of Welsh and provide in-person home contacts to the hard-to-reach areas across the rural geography due to limited public transport options, and patients’ inability to travel independently, therefore, this would be a critical factor for the service provision. The service will deliver a bereavement and anticipatory grief service to adults aged over 18 years to support a patients bereavement recovery and adjust to life after loss via various mediums including but not limited to, telephone, virtually via a suitable online platform and in person. The Health Board intends to award this contract under the competitive process, to a new provider.
