Description

North Yorkshire Council is looking to develop an Approved Provider List, to deliver services across the county for Children and Young People with disabilities. The potential client group to receive these services will be Children and Young People that are open to Children and Young Peoples services and Inclusion, with an assessed need for a short break service. The procurement will establish 3 new lots and enable packages of care to be commissioned and social care services to be put in place.

Lot Division

1 Community Based Support

Community Based Support Services are an essential part of Children or Young person’s ’s short breaks services, the Council requires a responsive service from Providers to ensure that children or young persons are supported to access social and leisure opportunities in their community to support them to continue in the care of their family or carers that love and care for them.

2 Overnight Short Breaks in a Residential Setting

Overnight Short Break Services are an essential part of Children or Young person’s ’s short breaks services, the Council requires a responsive service from Providers to ensure that children or young persons are supported to live in their own home and community with their family or carers that love and care for them.

3 Home Based Support

Home Based Support covers a range of activities, which may be put in place to support a Child or Young person in their own home. This may include the following core elements of care, Personal Care, Practical Support, Sitting Services and Overnight care.
