Housebound Annual Review Service for Patients with Long Term Conditions (LTCs) Services.
Total Quantity or Scope
The notice relates to Housebound Annual Review Service for Patients with Long Term Conditions (LTCs) health care services. This notice is an intention to award a contract to the existing provider following direct award process C. The Lifetime Value of the contract is £190,650 (£95,325 per annum). The contract term will be 1 year with the option to extend for a further 1 year at the sole discretion of the Authority. The contract will start on 01/04/2026 and end 31/03/2027. If the optional extension is utilised the contract end date will be 31/03/28.
