Total Quantity or Scope
The procurement is for the provision of discharge to assess (D2A) services through GP practices to support the safe and timely discharge of patients from hospital. The services comprise short-term post-discharge assessment, review, monitoring and follow-up in the community to identify patients' ongoing clinical, health and care needs and support appropriate onward care where required. The relevant authority is intending to award contracts to existing providers following Direct Award Process C. The approximate lifetime value of the contracts is £642,747. The services are intended to be provided from 1st April 2026 to 31st March 2027.

This is a Provider Selection Regime (PSR) intention to award notice. The awarding of this contract is subject to the Health Care Services (Provider Selection Regime) Regulations 2023. For the avoidance of doubt, the provisions of the Public Contracts Regulations 2015 do not apply to this award. The standstill period begins on the day after the publication of this notice. Representations by providers must be made to decision makers by 28th April 2026. This contract has not yet formally been awarded; this notice serves as an intention to award under the PSR. 1. Quality and Innovation • D2A service ensures that patients discharged from acute settings into Pathway 2 and Pathway 3 care home placements receive timely and clinically appropriate medical oversight. • The providers delivering this service are established GP practices and PCNs with a proven track record of delivering high-quality primary care services, aligned with both NHS England priorities and local system objectives. • Providers consistently meet service KPIs, ensuring timely medical oversight for patients discharged into care settings. • Structured clinical governance frameworks support quality assurance, patient safety monitoring, and adherence to CQC standards. • Remote consultation technologies, electronic patient records, and shared care plans have enhanced efficiency, accessibility, and real-time patient management. • Named GP leads provide ongoing continuity of care, ensuring that medical input remains consistent, reducing unnecessary interventions, and enhancing patient safety. 2. Value • The remuneration model ensures financial sustainability while delivering cost-efficiencies. • The service significantly reduces hospital readmissions, freeing up acute care capacity and generating system-wide savings. • The service leverages existing GP practice infrastructure, allowing for efficient case management within named provider groups, avoiding unnecessary duplication and excess costs. 3. Stage 3 - Provider Assessment on Likely Future Performance Assessment Against Criteria: 1. Quality and Innovation: • Providers are expected to sustain high-quality, responsive care, with continued compliance against KPI and CQC standards. • Further investments in Primary Care digital monitoring tools and electronic patient records will enhance service accessibility and efficiency. • Ongoing primary care clinical training and governance improvements will ensure continuous improvement in care quality. 2. Value: • The remuneration model will continue to provide stability while reducing pressures on acute services. 3. Integration, Collaboration, and Service Sustainability: • Providers will deepen collaboration with system partners, embedding enhanced multi-agency working models. • Service integration will ensure resilience, adaptability, and alignment with evolving NHS priorities. 4.
