This procurement follows an earlier market engagement exercise undertaken by the Trust to understand provider capability and inform the development of the service specification and commercial model. This engagement did not form part of a provider selection process and no provider was selected as a result of it.The Service provides digitally enabled proactive care for people with complex long-term conditions who are at increased risk of deterioration and avoidable admission.
